All performers and producers must complete the applicable forms before any content is produced. Download, print, sign, and retain a copy for your records.
Model Release & Consent Form
Required for all performers. Documents voluntary consent, grants content rights to the producing company, and specifies anonymity preferences.
Age Verification & ID Form
Required by federal law (18 U.S.C. § 2257). All performers must verify they are 18+ and submit a copy of a government-issued photo ID before production.
Production Agreement
Outlines the terms between performer and producer: scope of content, compensation, usage rights, anonymity measures, and cancellation policy.
Non-Disclosure Agreement (NDA)
Protects performer identity and confidential production details. Performer identity protections survive indefinitely.
